Official Transcript Request:
Any student past or present may request an official transcript to be sent to a specific institution.
A request form is available in the high school office, or you may make your request in writing and fax to 608-562-5755:
Please provide the following information:
1. *Your name while attending New Lisbon High School*
2. *Year of graduation or non-grad year (last year of attendance)*
3. *Phone number where the Guidance office may contact you if need be.*
4. *Name and exact address of the institution or business where the transcript should be sent.*
5. *Your signature.*
Official transcripts are sent as soon as possible during normal school in-session days.
To Make An Official Transcript Request Form:
A request form is available in the high school office, or you may copy and paste the 5 information steps
listed below to make your own form to fax or mail to the guidance ofice:

Unofficial Transcript Request:
Any student past or present may request an unofficial transcript for personal use. The request should include the five items previously mentioned. An unofficial transcript will not be signed or embossed and will be labeled as "unofficial".
At the present time, there is no fee charged for this service.

Official transcripts are sent as soon as possible during normal school in-session days and once every 10 days during summer break.
